Dynamic Forms Portal

The Dynamic Forms Portal is a simplified way for users to find fillable forms and view their forms activity in one consolidated landing page. Access to the Dynamic Forms Portal requires your Trinity login and will contain forms applicable to the logged-in user, whether you are faculty, staff, or student. A form can even be assigned to you, and the Dynamic Forms Portal is where you would access to complete it.

The Dynamic Forms Portal page includes the following::

  • Assigned To Me – A “To Do” list of forms that are assigned to you.
  • Pending Action – forms you are currently working on or need to be completed; you can also see your form use history from here.
  • Other Forms are frequently used forms sorted by departmental responsibility or form processing duties.

FAQs

How to open a form

  1. Navigate to the Dynamic Forms Portal at the following URL: https://trinitydc.dynamicforms.com.
  2. You will be prompted to enter your Trinity credentials.
  3. Within the Dynamic Forms Portal, go to the section called Other Forms.
  4. If you know the form’s name, you can use the search bar to type the name. As you type, the department with that name will show.
  5. Select the department by clicking anywhere on the name. The form you searched for will have a button: Fill out form.
  6. If you did not use the search features and clicked on the department, all forms in the department would show.

How to view and download your completed forms

  1. Navigate to the Dynamic Forms Portal at the following URL: https://trinitydc.dynamicforms.com.
  2. You will be prompted to enter your Trinity credentials.
  3. Within the Dynamic Forms Portal, go to the section called Pending Actions.
  4. Select View Forms History link from the bottom of the Pending Actions section.
  5. If you know the name of the form you are looking for, you can type in the name in the search field and click the search link. Otherwise, you can scroll down to find the form.
  6. Once you have located the form you want to view, print, or download, select the PDF or HTML icon to view.

My Dynamic Forms Portal does not look like that

  1. You selected the link to the Dynamic Forms Portal, but after you logged in, it took you to a different page, shown below:
  1. You arrived here because your account can process forms. If you want to fill out one of these forms, you can select the box with the arrow pointed up by the form you wish to use.
  2. If you do not see the form you are looking for, you can go to the Dynamic Forms Portal by selecting the My Forms dropdown list and Trinity Washington University User Portal.
