Student Examination Appointment Request

  • DIRECTIONS: Use this form to schedule a makeup exam. Please allow 2 business days for processing this form. If you submit your request less than 2 days prior to the requested exam appointment time, there is no guarantee that we will be able to proctor the exam for you. Your professor/instructor must submit the online Academic Test Request Form to complete your request.

    Testing Services Office Hours & Testing Hours:

    Staff will be available to consult with faculty and students regarding scheduling an exam Monday - Friday during our regular business hours.

    Staff will be available to administer SCHEDULED makeup exams during regular library hours, Monday –Saturday, but exams will be scheduled to end one hour before the library closes to allow time for processing paperwork.

Testing Services Office Hours & Testing Hours:
