Use this form to request makeup exams or special accommodation exams for students who need to schedule exams in the Testing Center.

All exams are by appointment only

This form is for faculty use. Students must request appointments for their exams online at discover.trinitydc.edu/academic-services/student-exam-appointment-request/.

Testing Services Office Hours & Testing Hours: Staff will be available to consult with faculty and students regarding scheduling an exam during our regular business hours, Monday – Friday.

Note to faculty/instructors: Students must use the online appointment request to schedule exams. Completed exams will be scanned & forwarded to you by email.
